Company Biography

Crossroads4Hope, A Network of Cancer Support, is transforming the cancer experience by closing the cancer care gap and reducing its burden on all people touched by the disease—the diagnosed, caregivers, family members, and the communities in which they live. As experts in psychosocial support and patient activation, our work is guided by social work principles rooted in social justice and valuing human dignity.

Individuals turn to us as THE safe space to restore their whole being and family. We activate and empower individuals to take control of their health and care by providing access to vital resources and a program of support, education, well-being, and hope.

We serve "previvors," patients, survivors, caregivers, family members, friends, and loved ones throughout the continuum of care, regardless of age, type of cancer, stage, or place of treatment, race, ethnicity, country of origin/citizenship, language, sexual orientation, insurance, or Veteran status. Our program pillars include Individual and Group Support; Navigation and Referrals; Decision Support; Educational and Nutrition Workshops; Mind Body Classes; Social Activities; and Financial Assistance. Individuals access our work across the United States remotely through video, digital, chat, and phone, and on-site at our New Jersey headquarters and our partner organizations in the community. Thanks to the generosity of our donors, no individual or family ever sees a bill.

MyGo2Support - Patient and Caregiver Support for the 21st Century

Crossroads4Hope is transforming how psychosocial support is delivered and scaled within healthcare and the community through our MyGo2Support program, powered by GoMo Health, which puts personalized social and emotional support, programs, and interventions directly into people's hands through a combination of intelligent digital design and Crossroads4Hope's "wrap-around" psychosocial programs. MyGo2Support is delivered directly to a mobile device without downloading an app.

MyGo2Support is a turnkey, scalable solution that seamlessly integrates into clinical workflows within healthcare, including clinical trials and corporate or government benefits programs. With over 97% of people owning a mobile device, MyGo2Support provides access to high-quality psychosocial support, resources, and interventions whenever and wherever needed. It integrates best practices in patient activation and psychosocial support with behavioral health technology. The program can lower direct and indirect cancer costs and include the patient's clinical team and caregiver.

MyGo2Support delivers ongoing and subtle weekly messaging appropriate to where a person is in their cancer journey at any point in time. Weekly, individuals complete a How Are You Doing? Survey® includes screeners for social determinants of health, the NCCN Distress Thermometer, and the PHQ2 or PHQ9. Based on scoring, the program triages individuals to professionals at Crossroads4Hope and/or a provider partner using MyGo2Support. Now, healthcare providers have real-time visibility to patients' emotional and social needs before becoming barriers to care. MyGo2Support also helps maximize limited staff by escalating those patients who need proactive support and intervention.

Supporting Individuals and Families

Together, we take on the cancer journey with all those impacted as they move through and beyond cancer. Our evidence-based framework and interventions address the health crisis caused by cancer; decrease distress and isolation; increase a sense of control; address health disparities; and increase awareness and access to psychoeducational resources, enabling patients and families to embrace healthy behaviors and gain hope, meaning, and a new attitude towards cancer. We fill people's "toolboxes" with knowledge, skills, and resources needed throughout their lifetime. Ultimately, we give people the confidence to self-advocate and take control of their health and well-being.

Supporting Communities – The Gladiator Program – Activating Health Champions in Vulnerable Communities

Crossroads4Hope is leveraging public health resources to help "close the gap" and create new points of entry to our program, bringing the most relevant aspects of our work to vulnerable and underserved communities, embedding our program as far upstream as possible in the places where people live and receive their care, to be able to reach people as early as possible -- even before being diagnosed to ensure the most significant impact of our efforts. This approach enables us to increase awareness and normalize conversations around cancer, allowing individuals to explore cancer and its intersection with their values. We have tailored interventions for implementation starting with the following communities: African, Afro-Caribbean, African Americans, Hispanic/Latinos, Children and Families, Young Adults, LGTBQ+, and Veterans. Our public health team is building trusted partnerships and bringing wrap-around resources to the community, including educational workshops covering cancer, the cancer experience, nutrition, and how to care for your mind and body. We are reaching "previvors" through collaborations with cancer screening partners who value the integration of our psychosocial support, programs, navigation and referral resources, and food box distribution programs to provide timely access to care. To scale our effort, we are mobilizing a network of professionals and members within trusted partner organizations in vulnerable communities who are often the first to become aware when someone in their community is affected by cancer.

Our newly launched Gladiator Program will mobilize and empower "Champions" who normalize conversations around cancer, acting as "first responders" to support the immediate needs of people affected by cancer in their community, guiding those affected in ways that allow them to remain informed and actively involved in decisions about their care across the cancer continuum to ensure that they can achieve the best possible health outcomes. Activated Health Champions can now provide timely referrals to our social work, child life, and nutrition professionals for nutritional counseling, support, navigation, referral resources, and financial assistance; and help prepare individuals to participate in shared treatment decision-making with their doctor.

To address the lack of trust in the healthcare system and clinical trials, our "Diversity Matters: Advancing Cancer Treatments Through Increased Clinical Trial Participation" video helps Champions initiate meaningful conversations in the community to enable individuals to self-advocate. Additionally, they will enroll individuals into our MyGo2Support digital, direct-to-mobile program that provides 24/7 support, screenings for social needs, the risk for distress and depression, and a library of resources that can be accessed when and where it is needed most.
